Extra Text Boxes to complete when a post is made.

Detailed description of the tasks:

This new forum will be for a small Closed Group.
If someone can give me a fixed price to complete the tasks listed below, as well as when it will be completed?

TASK ONE:
Fully set up the most recent PHPBB forum on a server, including the Domain.

TASK TWO:
1, When a topic is started, I would like nine text entry boxes to appear below the “topic title” field.
These nine text fields will be required to be completed.
Then, when a topic is selected to be read, it will show the topic title as normal, but it will show the nine rows of text entries, entered below the title.

2, When a post is made, it will the posted in numerical order and include the number in the listing.

TASK THREE:
There should be two new Search option choices.

• Search by “post number”
  Search by “new entry fields”

I would like this work to be done but keep in mind, that new versions of phpbb, php etc. and how will they affect the work done or will it be a simple update?
